What happened—and what I understood
A record can leave room for uncertainty.
An observation, a report from someone else, and your interpretation are different kinds of material. Keeping them separate can make a note more useful later.
You might write: what I noticed; what someone said; what I understood; what I do not yet know. These are optional headings, not a requirement to document your life.
You can correct an account without pretending the earlier version never existed. Keep private material private, and avoid assigning a diagnosis or motive from a fragment of behavior.
